How do conjoined twins occur/form?

I found a random video about these two conjoined twins named Abby and Brittany Hensel. That got me to wondering...how exactly does this happen? Like, how do conjoined twins get that way in the womb? Is it some sort of mutation, or something? I'm not being rude. I was just wondering how exactly this happens, or what causes it?
